Dear Applicant,

Congratulations on the successful submission of your online application for the City of Friendswood. The next step in the process is to submit a completed background packet.

Failure to submit all documents and a complete and accurate Personal Information Form (PIF) will result in your application being closed and rejected.

The following is a list of the required documents that must be submitted. Additional forms and/or documents will be requested during the application process.

The PIF is a fill in form document that can be completed using your computer.

1. PIF (MUST be Typed) (Signed in front of notary)
2. PIF Instruction Sheet-(Signed and dated)
3. Release/Waiver of Liability Form (signed and filled out in front of a notary)
4. Integrity Warning Form (Signed in front of a notary)
5. Birth Certificate (copy) – UNACCEPTABLE
6. Driver’s License (copy)
7. Marriage Certificate (copy) – UNACCEPTABLE
8. Copy of GED (if applicable)
9. All High School Transcripts (sealed copy) – UNACCEPTABLE and a copy of your High School Diploma. – UNACCEPTABLE
10. All College Transcripts (sealed copy) – UNACCEPTABLE
11. Dissolution of Marriage Document (copy) – UNACCEPTABLE
12. Military Discharge papers (DD214 Long Form)
13. Proof of Financial Responsibility (copy of motor vehicle insurance) – UNACCEPTABLE
14. Telecommunicator License (if applicable)
15. Experian Credit Report (if there are negative entries, provide letter with explanation) – UNACCEPTABLE
16. Social Security Card (copy)
17. Previous two law enforcement performance evaluations (if applicable) – DOES THIS APPLY TO OFFICER CANDIDATES?
18. Color Photograph – UNLAWFUL AND UNACCEPTABLE AND DISGUSTING
19. Certificate of Naturalization (if applicable)

Be very thorough and completely honest in the Personal Information Form. If there are boxes which ask for full details about an event/incident be very thorough in your answers. If you need more room to explain a situation, you can always submit an attachment on a word document. Just be sure and reference what question you are referring to. Here is some additional information:

• For previous jobs, do your best to list the requested coworkers’ information. We understand that on older jobs you may not have all the information. If all you can remember is a name then list the name. Complete this section to the best of your recollection and if you can’t recall all requested information then state that in the box.
• All official high school and college transcripts can be mailed directly to me from your schools. If you are having your transcripts sent directly to me be sure and notate that in your packet on the PIF instruction form.
• Official transcripts are needed from every high school and every college you ever attended regardless if you received credits or not.
**Due to most schools and colleges being closed, we will accept copies of unofficial transcripts until you are able to provide official documents. If you’re unable to obtain copies of unofficial transcripts, please contact me. You can have the schools/colleges mail the official transcripts directly to me at the police department if you would like. If you do not have official transcripts at the time packets are due, you can go ahead and turn them in anyway. Please notate in your packet that the official transcripts are pending so I will know you are working on it**
• The credit report submitted must be an Experian credit report. The entire credit report (which shows monthly account history) must be submitted and not just the cover page or score.

Transunion and Equifax credit reports will not be accepted. Be sure and review your credit report and provide a detailed explanation if you have any negative entries on the report such as late payments, delinquent accounts, charge offs, etc. – UNBELIEVABLE
• Please pay close attention to the Creditor/Bills section in the Personal Information Form and what information is being requested. Many applicants fail to follow directions on this section. Don’t be disqualified for failing to follow directions. The last question in this section asks for total monthly obligations and total monthly income. This information must be provided. – NONE OF HIS DAMN BUSINESS.
• A photocopy of your original birth certificate will suffice. – UNACCEPTABLE
• Previous law enforcement evaluations only apply to those applicants who have held or currently hold employment for a law enforcement agency.

The packet can be dropped off in person 24 hours a day at the Friendswood Police Department located at 1600 Whitaker Dr., Friendswood, TX 77546. Put all the information into a large envelope to the attention of Sergeant Brechtel. The personnel at the Records or Dispatch window will ensure that I receive it. You can also mail it to me. I recommend using a delivery confirmation type service so you know I received it.

Remember the Personal Information Form (PIF), Integrity Warning Form and the Release/Waiver of Liability need to be signed in front of a notary. We have notaries available at the police department. They will be happy to notarize these forms for you.

Once all packets are received they undergo a thorough screening process. Please be patient during the screening process, as this process usually takes several weeks. Those best qualified will proceed to the next step in the hiring process.
